Please bring your completed physical with you to your scheduled appointment.
Physical forms can be found on the WVSSAC Website, under the Sports Medicine Tab: Physical Form and in the main office. You will be turned away if your form is not completed, including parent or guardian signature. Do not participate in physical activity or drink caffeine 2 hours prior to your appointment. Questions? Information was sent home with 4th grade students. Any PCMS student that is interested in joining band for the 2025-2026 school year, see Mr. Jones for more information.
