Memorial Health System Department of Sports Medicine will be providing 2026-2027 Sports Physicals for current 6th-11th grade students at no cost on Friday, June 5th; 1:00 p.m. - 6:00 p.m. in the St. Marys High School gymnasium.
Physicals are by appointment only. Book Appointment
Please bring your completed physical form & insurance card with you to your scheduled appointment. Hard-copy physical forms will be available at the check-in desk. You will be turned away if your form is not completed, including parent or guardian signature. Do not participate in physical activity or drink caffeine 2 hours prior to your appointment.
